Community-Based Hypertension Care Pilot Project in Liberia
.png)
CAdHSI, in collaboration with Johns Hopkins University and Wellness Partners Clinic, is implementing a six-month pilot study to improve hypertension detection and control in Monrovia, Liberia. The project trains community health nurses to deliver guideline-based, nurse-led hypertension care through mobile clinics located in high-traffic communities.
Using both medication and lifestyle counseling aligned with World Health Organization protocols, the study evaluates how this community-driven approach can reduce blood pressure, improve patient trust and adherence, and strengthen local capacity for chronic disease management. By empowering frontline nurses and integrating digital tools for data collection and monitoring, the project aims to create a scalable model for equitable, community-centered hypertension care in resource-limited settings.
